Parish Registration Form

Thank you for your interest in St. John’s Parish.

Since registration is our only means of certifying parish membership, we encourage all parishioners to register. There are multiple ways to register. One, which we suggest, is visiting the Parish Center and registering in person. In addition, you can download the registration form here (PDF Document) and drop it off at the Parish Center in person or mail it to:
